NTSB investigating fatal accident at Powell Street Station

BART is investigating a fatal accident that occurred at Powell Street Station at 3:16pm on Monday, September 13, 2021.

NTSB investigators are also investigating.

The tragic incident began on the platform. A female was on the platform with a dog that was tethered to her waist. She boarded a Dublin/Pleasanton bound train with the dog and then at the very last second, right as the doors were closing, she fully off boarded the train, clearing the doors, and the dog did not. She appeared to be waving at someone. The train departed with the dog inside the train and the woman was dragged and pulled into the track. The dog was not injured. There was no visual indication the dog was a service animal, but it is under investigation.

BART Police have reviewed surveillance footage and interviewed witnesses. BART’s Chief Safety Officer is working with NTSB investigators and BART Police and the investigation is on-going.

“This is a tragic loss of life, and we are following all safety protocols,” said BART Spokesperson Alicia Trost.

Media inquiries should be directed to the NTSB.
